Job Description

Signature HealthCARE is seeking a compassionate and clinically skilled Speech Language Pathologist to join our Healthcare & Medical Services team in delivering resident-centered rehabilitation services. In this role, you will provide comprehensive evaluation, treatment, and management of speech, language, cognitive-communication, and swallowing disorders for adult and geriatric patients in our care. Working within a collaborative, family-based healthcare environment, you will partner closely with an interdisciplinary team of nurses, physicians, occupational and physical therapists, social workers, and families to develop and implement individualized treatment plans that enhance functional communication, safety, and overall quality of life. As a Speech Language Pathologist with Signature HealthCARE, you will conduct thorough clinical assessments, develop measurable goals, and deliver evidence-based therapy tailored to each resident’s needs, cultural background, and personal preferences. You will provide direct treatment for a range of conditions, including aphasia, dysarthria, cognitive-linguistic impairments, voice disorders, and dysphagia, using both traditional interventions and innovative approaches aligned with current best practices. You will also play a key role in patient and family education, training caregivers in communication strategies, safe swallowing techniques, and compensatory methods that promote independence and dignity. Daily responsibilities include accurate and timely documentation in the electronic medical record, participation in care conferences and team meetings, coordination of services across disciplines, and adherence to all regulatory, ethical, and professional standards of practice. You will monitor resident progress, adjust goals and treatment plans as needed, and communicate proactively with physicians, nursing staff, and families regarding clinical status and recommendations. You may also mentor support staff and participate in in-service training to promote a culture of continuous learning and clinical excellence.
