Job Description
Lake Centre for Rehab is seeking a skilled and compassionate Physical Therapist to join our Healthcare & Medical Services team in providing comprehensive rehabilitative care to a diverse patient population.
As part of a collaborative, patient-centered environment, you will deliver high-quality physical therapy services focused on restoring function, reducing pain, and maximizing independence for individuals with orthopedic, neurologic, post-surgical, and other complex conditions.
In this role, you will be responsible for performing thorough evaluations, interpreting clinical findings, and developing evidence-based, individualized treatment plans that align with each patient’s goals and lifestyle.
You will implement a variety of interventions, including manual therapy, therapeutic exercise, gait and balance training, functional mobility training, and the appropriate use of therapeutic modalities, while closely monitoring patient progress and adjusting plans of care as needed.
You will provide clear, empathetic education to patients and their families on diagnoses, treatment approaches, home exercise programs, and strategies to prevent re-injury, ensuring that each patient is actively engaged in their own recovery.
Working closely with an interdisciplinary team of therapists and referring providers, you will participate in collaborative care planning, timely communication, and coordinated transitions of care.
Accurate, timely documentation in the electronic medical record, adherence to clinical and regulatory standards, and commitment to best practices and outcome-driven care will be central to your daily work.

Active PT license in good standing in the state of employment
Experience requirements vary by employer.
BLS typically required.
Ability to stand for long shifts, lift up to 50 lbs, and assist with patient transfers.
