Job Description
Stellar Senior Living is seeking a compassionate and motivated Physical Therapist (PT) to join our Healthcare & Medical Services team, delivering high-quality, resident-centered rehabilitative care in a vibrant senior living community. As a Physical Therapist with Stellar Senior Living, you will play a key clinical role in evaluating and treating residents to improve mobility, reduce pain, enhance function, and support overall wellness. Drawing on a deep commitment to dedicated, compassionate service, you will design and implement individualized treatment plans that reflect each resident’s goals, medical status, and functional abilities while working within a supportive, interdisciplinary environment. In this role, you will complete thorough physical therapy evaluations, including assessments of strength, range of motion, balance, gait, and functional mobility. Using your professional judgment and clinical skills, you will develop targeted treatment plans that may include therapeutic exercise, gait and transfer training, neuromuscular re-education, balance and fall-prevention programs, and patient and caregiver education. You will regularly monitor outcomes, document progress, and adjust interventions to maximize each resident’s potential for independence. Collaboration is central to this position. You will work closely with nurses, physicians, caregivers, and other healthcare professionals to coordinate care, contribute to interdisciplinary care conferences, and support effective care and discharge planning. You will also serve as an educational resource for residents, families, and staff, providing instruction on safe mobility techniques, home exercise programs, and proper use of assistive devices to promote safety, confidence, and quality of life. Strong communication skills, clinical reasoning, time management, and a team-oriented mindset are essential, as is a genuine desire to support residents in achieving their highest level of functional independence.
