Job Description

Life Care Centers is seeking a compassionate, patient-focused Physical Therapist to join our Healthcare & Medical Services team.

In this role, you will provide evidence-based rehabilitation services to primarily senior and post-acute patients within a supportive, family-oriented environment.

You will complete comprehensive physical therapy evaluations, develop individualized treatment plans, and deliver skilled interventions to help residents regain function, improve mobility, manage pain, and maximize independence.

Working in close collaboration with an interdisciplinary team of nurses, physicians, occupational and speech therapists, and social workers, you will contribute to patient-centered care plans that prioritize safety, dignity, and quality outcomes.

Your daily responsibilities will include therapeutic exercise and balance training, gait and transfer training, manual therapy, neuromuscular re-education, and functional mobility activities tailored to each patient’s goals and needs.

Accurate and timely documentation in the electronic health record, including evaluations, progress notes, and discharge summaries, is essential to support regulatory compliance and optimal reimbursement.
