Job Description

Please note that this role is per diem with a rate of $130 per visit. Responsibilities Evaluate patients’ functional abilities and home environments to identify needs and safety concerns Develop personalized treatment plans and communicate them to physicians and the care team Teach and retrain patients in daily living skills affected by illness or injury Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices Maintain accurate clinical records and promptly report changes in patient condition Coordinate with the broader care team to ensure seamless, patient-centered care Educate family members and caregivers on the patient’s treatment approach Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service Support discharge planning as patients transition out of home health care Perform other duties as required for patient services Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association Registered with the National Registration Examination of the American Occupational Therapy Association Active California Occupational Therapy license Minimum 2 years of clinical OT experience Current CPR certification with AHA Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes Strong communication and organizational skills Flexible Schedule Ability to Convert to Full-Time Based On Performance If Interested Paid Sick Days Paid Bereavement Leave Skills Training & Development Company Social Events Mileage Reimbursement for Patient to Patient Visits Referral Bonus
