Compensation
- Visit Rates with Mileage Reimbursement: $63.00 - $100.00 per visit
- Visit Rates without Mileage Reimbursement: $68.00 - $105.00 per visit
- Discharge Without a Visit: $5.00 - $10.00 per non-visit discharge
- Transfer Without a Visit: $10.00 per non-visit transfer
- Orientation and In Service Hours: $30.00 per hour
Non taxed mileage reimbursement is paid at $0.50 per mile after 30 miles per day Part-Time and Full-Time Benefits (opportunity for growth): 401K with 4% Match with Immediate Vesting (eligible after one year of employment) PTO Paid Weekday Holidays Non-Taxed Mileage Reimbursement Group Health Insurance Professional Liability Insurance Job Description: Provides skilled physical therapy in accordance with the physician’s plan of care. Accepts only patients referred by the patient care manager or designee. Assists the physician in assessing the patient’s functional level by applying appropriate tests and assists in the development and revision of the plan of care, including OASIS assessments and goal setting as appropriate. Treats patients to relieve pain, develop and restore function and maintain maximum performance, using appropriate physical therapy treatments, modalities and procedures as stated in the physical therapy plan of care. Identifies barriers to desired outcomes. Observes, records and reports the patient’s reaction to treatment and any change in the patient’s condition to the physician and/or patient care manager and other staff involved. Instructs and advises patient, family and other health team personnel including, when appropriate, the physical therapy assistant and home health aides, in the phases of physical therapy in which they may assist the patient; instructs all of the above in the use of prosthetic and orthotic devices and durable medical equipment, as appropriate. The goal is to teach and promote disease prevention and injury management for eventual patient & caregiver independence, utilizing agency materials as appropriate. Requirements: Active Michigan Physical Therapy license Reliable transportation Valid driver's license and auto insurance in your name as a driver Home health care experience preferred Degree of Travel: Home visits and office meetings as indicated. Must participate in necessary in-service programs. Must have reliable transportation and agency-required auto insurance.
