David's Medical Center seeks a Home Health Physical Therapist to provide evidence-based rehab services to patients in their homes across the Austin area.
Responsibilities include evaluating functional status, developing and updating individualized care plans, delivering therapeutic interventions, educating patients and caregivers, and collaborating with multidisciplinary teams.
You’ll document in the EMR, ensure safety in the home environment, and support smooth transitions of care.
