Job Description
Signature HealthCARE is seeking a compassionate and skilled Speech Language Pathologist to join our Healthcare & Medical Services team within our Healthcare & Medical Services department. In this resident-centered role, you will provide high-quality assessment, diagnosis, and treatment of speech, language, cognitive-communication, and swallowing disorders for a diverse caseload of adult and geriatric patients. Working in a collaborative, family-based care environment, you will partner closely with nurses, physicians, rehabilitation therapists, social workers, residents, and families to develop and implement individualized, evidence-based treatment plans that align with each patient’s goals, preferences, and overall plan of care. As a Speech Language Pathologist at Signature HealthCARE, you will conduct comprehensive evaluations using standardized assessments and clinical observation, identify communication and swallowing needs, and deliver person-centered interventions that promote functional communication, safe swallowing, and improved quality of life. You will provide direct therapy services, train and educate patients, families, and caregivers on compensatory strategies, and recommend appropriate assistive technology or augmentative and alternative communication (AAC) when indicated. Your role will also include dysphagia management, including clinical swallowing evaluations and participation in instrumental assessments in collaboration with the broader clinical team, as available. You will document all assessments, goals, progress notes, and discharge summaries accurately and timely in the electronic health record, ensuring regulatory and organizational compliance. Active SLP license in good standing in the state of employment

Ability to stand for long shifts, lift up to 50 lbs, and assist with patient transfers.
