Job Description
CarePartners seeks a dedicated Physical Therapist – Home Care to provide one-on-one rehabilitation services to patients in their homes.
You will evaluate functional mobility, develop and implement individualized treatment plans, and collaborate closely with an interdisciplinary team to promote independence, safety, and quality of life.
