Job Description
Stellar Senior Living is seeking a compassionate and skilled Physical Therapist (PT) to join our Healthcare & Medical Services team, supporting residents across our senior living community. In this role, you will play a vital part in helping older adults maintain or regain their independence, mobility, and overall quality of life within a warm, supportive environment. As a Physical Therapist with Stellar Senior Living, you will perform comprehensive evaluations of residents’ functional abilities, including strength, range of motion, balance, gait, and endurance. You will use your clinical expertise to develop individualized, evidence-based treatment plans aimed at improving functional mobility, reducing pain, preventing falls, and enhancing participation in daily activities. Through therapeutic exercise, gait and balance training, neuromuscular re-education, and manual techniques where appropriate, you will work one-on-one with residents to help them progress toward their goals. You will collaborate closely with our nursing team, physicians, and the broader interdisciplinary care group to ensure a coordinated approach to resident care. Regular communication and participation in care conferences will allow you to advocate for residents, adjust treatment plans as their needs change, and contribute clinical insights that support the highest possible standards of care. You will provide education to residents, families, and caregivers on safe mobility, use of assistive devices, home exercise programs, and fall-prevention strategies to promote ongoing success beyond the therapy session. Accurate, timely documentation of assessments, treatment sessions, progress notes, and care plans is an essential part of this position, as is adherence to state and federal regulations and professional standards of practice. You will help uphold Stellar Senior Living’s commitment to high-quality, person-centered care by maintaining detailed records that reflect each resident’s progress and response to therapy. Requirements
Active PT license in good standing in the state of employment
1+ year of clinical experience preferred; new grads considered for residency tracks.
BLS required. ACLS preferred for hospital roles.
Ability to stand for long shifts, lift up to 50 lbs, and assist with patient transfers.
