Job Description

We are seeking a dedicated and experienced Part Time Case Manager to join our TEAM! Monday-Friday 9am-1pm (Not a Remote Position) Advanced Recovery Systems is an integrated behavioral healthcare management company dedicated to the treatment of addiction, substance abuse, and mental health issues. We put behavioral health front and center, providing assistance to people with substance abuse issues, addictions and mental health concerns. The Case Manager is responsible for providing comprehensive case management services with a primary focus on discharge planning and individualized aftercare coordination, in compliance with applicable State, Federal, and accreditation standards. This role conducts discharge needs assessments, develops and coordinates individualized aftercare plans, and works directly with patients to schedule follow-up appointments for ongoing care, including referral to ARS outpatient services for continued treatment when appropriate. The Case Manager collaborates closely with the multidisciplinary treatment team to ensure aftercare plans align with clinical recommendations and patient needs and works directly with business development, referents, and payors to coordinate continued care services. This position supports patients remaining engaged in treatment by assisting with employment- and legal-related needs, crisis situations, and other barriers to successful treatment participation and discharge. The Case Manager works in close partnership with Financial Case Managers to address financial considerations that may impact discharge planning and continuity of care and may provide coverage for financial counseling functions as needed to support patient flow and operational continuity. Core Job Duties: Conducting comprehensive discharge and aftercare assessments; developing and coordinating appropriate referral and aftercare plans; and ensuring alignment with individualized treatment plans. Follows treatment plans, contributes timely treatment plan updates, and completes accurate discharge summaries to support continuity of care. This role requires a strong commitment to patient-centered care, compassion, accountability, and ethical practice, as well as collaboration, flexibility, adaptability to change, openness to supervisory feedback, and a continuous focus on quality, compliance, and improvement Complete initial case management assessments within 72 hours of admission and ongoing weekly discharge needs assessments, developing individualized aftercare plans that include crisis planning as clinically indicated. Prioritize referral to ARS outpatient services for step-down levels of care when clinically appropriate to support continuity of treatment within the ARS continuum. Work directly with patients to coordinate and schedule aftercare appointments and referrals that support ongoing treatment and recovery. Coordinate and schedule follow-up aftercare appointments within seven (7) days of discharge, including MAT provider appointments when applicable, and ensure timely referrals to appropriate outpatient or community-based services to support continuity of care. • Collaborate closely with Business Development, referents, payors, and community providers to coordinate continued care needs and facilitate timely step-down placements. Maintain open and ongoing communication with the clinicians and multidisciplinary treatment team regarding discharge planning, aftercare progress, and patient needs. Assist patients in remaining engaged in treatment by addressing employment, legal, and other psychosocial barriers. • Assist patients with employment-related documentation and coordination, including FMLA, short-term disability, state paid leave programs, return-to-work documentation, and other employment-related requirements that support treatment participation and successful discharge. Assist patients with COBRA election processes, including verification of coverage activation and confirmation of required premium payments, when applicable. Ensure aftercare planning aligns with the individualized treatment plan and organizational standards. Ensure Discharge Folders are complete, accurate, and patient-ready at the time of discharge, including individualized aftercare resources, referrals, scheduled appointments, the completed Case Management Discharge Plan (CMDP) report, and copies of required documentation such as letters of presence, letters of completion or incompletion, and return-to-work letters. Work in coordination with Financial Case Managers to address financial considerations that may impact discharge planning and continuity of care. Provide coverage for Financial Counseling functions as needed, including patient financial education, coordination with RCM/Admissions, and assistance with required financial documentation. Bachelors’ Degree in health-related field preferred. Minimum high school diploma. Two years' clinical experience in the medical and/or behavioral healthcare field. Benefits begin on the 1st day of the month following date of hire.
