Job Description
Eastside Rehabilitation Hospital is seeking a dedicated Physical Therapist to join our Healthcare & Medical Services team, providing comprehensive rehabilitation care to patients recovering from illness, injury, or surgery. In this role, you will work with a diverse caseload of adults who may present with neurological, orthopedic, cardiopulmonary, and other complex medical conditions. Drawing on your clinical expertise, you will perform thorough evaluations, establish functional goals in collaboration with patients and families, and design individualized, evidence-based treatment plans that promote mobility, strength, balance, and independence. As a Physical Therapist at Eastside Rehabilitation Hospital, you will provide a full spectrum of therapeutic interventions, including manual therapy, therapeutic exercise, gait and transfer training, neuromuscular re-education, balance and coordination activities, and post-surgical rehabilitation programs. You will utilize appropriate rehabilitation equipment and modalities while carefully monitoring patient response and adjusting treatment plans to optimize outcomes. Accurate and timely documentation in the electronic medical record will be an essential part of your daily responsibilities, ensuring continuity and quality of care. You will be an integral member of a compassionate, patient-centered, interdisciplinary team, collaborating closely with physicians, nurses, occupational therapists, speech therapists, case managers, and other healthcare professionals. Together, you will participate in care planning, case conferences, discharge planning, and patient education to support safe transitions and long-term success at home or in the community.
