Job Description
CenterWell Home Health is seeking a compassionate and autonomous Home Health Physical Therapist to deliver high-quality, evidence-based rehabilitative care in the comfort of patients’ homes. In this role, you will travel to patients’ residences to evaluate functional mobility, develop individualized treatment plans, and provide hands-on therapeutic interventions that help patients regain independence, improve safety, and enhance overall quality of life. You will perform comprehensive physical therapy assessments, establish measurable goals, and implement treatment programs including therapeutic exercise, gait and balance training, transfer training, pain management strategies, and patient-specific home exercise programs. You will regularly reassess progress, update care plans, and coordinate closely with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary team to ensure continuity and excellence in care. As a key clinical professional at CenterWell, you will educate patients and family caregivers on safe mobility techniques, fall prevention, proper use of assistive devices, and strategies to manage chronic conditions in the home setting. You will document all care according to regulatory and agency standards using electronic medical records, ensuring timely, accurate, and compliant charting. You will help uphold CenterWell’s culture of compassion, integrity, and continuous improvement by adhering to best practices, participating in case conferences, and contributing to quality and performance initiatives.
