Job Description
The expected salary range for this position is based on several factors including market, funding, candidates experience level, skills, training, and related knowledge. Our ranges are developed and published as good faith estimates for the position. Salary Range: $58,656 - $62,049 | Office benefit package (Parking/Remote Expenses): $130/Month Sign On

Are you comfortable moving from program to program? Do you have experience or want to gain experience in any of the following programs? If so, this opportunity may be for you! The Float Practitioners will work in various programs and provide support, rehabilitation, and treatment to individuals served. Floats for Case Management Services support: Assertive Community Treatment (ACT) Targeted Case Management (TCM) Homeless Services Community Treatment Team Waivered Services Case Management Homeless Services and more General schedule is Mon-Fri 9am-5pm. Floats for Residential Services support: Intensive Residential Treatment Services (IRTS) Crisis Stabilization Program Provide supervisory support to Residential Support Services group home(s) Floats hired for this program are fully dedicated to this program General schedules are determined by program needs Job Duties & Responsibilities Develop and implement the treatment plan that is based on the diagnostic and functional assessment which is approved by the clinical supervisor. Provide courteous and respectful care to all client. Develop an individualized emergency support plan coordinated with community emergency support services. Provide direct services to the residents. Teach and develop independent living skills (relaxation, conversation skills, housekeeping, budgeting, etc.) for the clients. Identify individuals prone to having behavior emergencies and assists in developing additional programming appropriate for their needs. Provide education to clients regarding the symptoms, course and treatment of their illness. Responsible for knowledge and implementation of service and behavior plans. Collaborate with, support and provide education to family members and others interested in the client’s treatment and welfare when warranted. Make appropriate / culturally sensitive referrals and assist clients in gaining access to needed services. Assist clients with all aspects of psychiatric rehabilitation and all aspects of obtaining and maintaining housing. Will assist clients with making and keeping appointments and any other tasks that are required through the client’s recovery process. File all legally required reports with Vulnerable Adults, the Ombudsman and Child Protection. Utilizes crisis assessment tools for suicidal and aggressive behavior. Help to defuse and de-escalate any potential behavior emergencies and assists in developing additional programming appropriate for the client’s needs. Attend team meetings as scheduled. Develop appropriate therapeutic relationship with clients. Attend and participate in all scheduled team meetings. Must be able to transport clients in the community. Complete all paperwork, reports, and charting contemporaneously and in an organized manner. Participate actively in on-going professional growth and development, maintain appropriate professional behavior, and participate in appropriate supervision. Maintain a pattern of regular work hours to meet program needs as determined by the Program Director. Other duties as assigned.
