Implement therapeutic interventions to improve mobility, strength, balance, endurance, and functional independence; monitor progress and adjust as needed. Educate patients and caregivers on exercises, safety, gait training, activities of daily living, and home exercise programs. Document assessments, treatment provided, progress notes, and discharge planning in the medical record; ensure regulatory and payer compliance. Collaborate with physicians, nurses, and other care team members to coordinate care and align goals with patient milestones. Requirements Active physical therapy licensure in all applicable states of practice; for this assignment, Oregon licensure is required. Doctor of Physical Therapy (DPT) degree is required, if licensure was obtained on or after January 2016, or foreign equivalent. AHA BLS certification. Home health experience.
