Job Description

Life Care Centers is seeking a compassionate and skilled Speech Language Pathologist to join our Healthcare & Medical Services team.

In this role, you will provide evaluation and treatment of speech, language, cognitive-communication, and swallowing disorders for senior residents in a rehabilitation-focused, long-term care environment.

Working within a patient-first, family-focused culture, you will conduct comprehensive assessments, develop individualized, evidence-based treatment plans, and deliver direct therapy services that support functional communication, safe swallowing, and improved quality of life.

You will collaborate closely with an interdisciplinary team—including nursing, physical and occupational therapy, physicians, social services, and families—to coordinate care, set meaningful functional goals, and optimize outcomes for each resident.

Responsibilities include performing standardized assessments, documenting findings and progress in the electronic medical record, providing patient and caregiver education, participating in care conferences, and contributing to discharge planning and home programming as appropriate.

You will uphold high clinical standards, follow all regulatory and ethical guidelines, and actively support a culture of safety, dignity, and respect for every resident.
