Job Description
Synchrony Rehab is seeking a dedicated and compassionate Physical Therapist to join our Healthcare & Medical Services team, delivering high-quality, patient-centered rehabilitation services across a variety of care settings. In this role, you will use your clinical expertise to evaluate patients, develop individualized treatment plans, and implement evidence-based physical therapy interventions that promote optimal recovery, functional independence, and overall well-being. As a Physical Therapist with Synchrony Rehab, you will perform comprehensive evaluations to identify impairments, functional limitations, and patient goals, then design and adjust plans of care tailored to each individual. You will provide hands-on treatment, including therapeutic exercise, manual therapy, gait and balance training, and functional mobility training, while closely monitoring progress and modifying interventions as needed. You will educate patients, families, and caregivers on conditions, home exercise programs, and strategies to maximize safety, mobility, and quality of life. You will work collaboratively within a highly supportive, team-oriented culture that values integrity and innovation. Partnering with occupational and speech therapists, nurses, physicians, and other members of the healthcare team, you will contribute to coordinated care plans and participate in case conferences and interdisciplinary meetings to enhance patient outcomes. Accurate and timely documentation within the electronic medical record will be an essential part of your daily practice, ensuring continuity and quality of care.
