Job Description
CarePartners is seeking a compassionate Physical Therapist for Home Health within our Healthcare & Medical Services department.
In this role, you will provide in-home evaluations, develop individualized treatment plans, and deliver hands-on therapy to improve mobility, function, and quality of life for our patients.
You’ll collaborate with an interdisciplinary team, document outcomes, and educate patients and families.

Ability to stand for long shifts, lift up to 50 lbs, and assist with patient transfers.
