Job Description

Stellar Senior Living is seeking a compassionate and motivated Physical Therapist (PT) to join our Healthcare & Medical Services team, supporting residents in our vibrant senior living community. As a key clinical partner in our mission to enhance lives and foster meaningful communities, you will provide high-quality, person-centered physical therapy services to older adults in independent living, assisted living, and memory care settings. In this role, you will complete comprehensive evaluations to assess each resident’s strength, balance, mobility, pain level, and functional abilities. Drawing on your clinical expertise and passion for geriatrics, you will design and implement individualized treatment plans that promote safety, independence, and overall well-being. Your interventions may include therapeutic exercise, gait and balance training, functional mobility activities, and manual therapy, all tailored to each resident’s goals and medical needs. Working within a supportive, interdisciplinary environment, you will collaborate closely with nursing staff, physicians, occupational and speech therapists, and other care team members to coordinate holistic, resident-centered care. You will participate in care conferences, fall prevention initiatives, and quality improvement projects that reflect Stellar Senior Living’s commitment to compassionate service and continuous growth. Education and support are central to this position. You will teach residents and their families or caregivers how to safely perform home exercise programs, use assistive devices, and incorporate functional strategies into daily routines. You will also provide in-service training and ongoing guidance to community staff on safe transfers, mobility techniques, and carryover of therapy recommendations, helping to create a safer, more supportive environment for residents and team members. The Physical Therapist is responsible for thorough and timely documentation of evaluations, daily treatment notes, progress reports, and discharge plans that meet all professional, regulatory, and payer requirements. You will regularly reassess residents’ progress, adjust treatment plans as needed, and communicate changes in condition to the broader care team, always advocating for the resident’s best interests.
