CarePartners of Connecticut is seeking a dedicated Physical Therapist for our Home Health CarePartners team within the Healthcare & Medical Services department.
In this role, you will deliver high-quality, evidence-based physical therapy services to patients in their homes across the community, supporting the company’s mission to provide coordinated, patient-centered care.
Working closely with referring physicians, nurses, care managers, and other allied health professionals, you will perform comprehensive evaluations, develop individualized plans of care, and implement therapeutic interventions that promote mobility, safety, pain reduction, and functional independence.
You will educate patients and caregivers on home exercise programs, equipment use, and fall-prevention strategies, ensuring that each plan is tailored to the patient’s goals, health status, and home environment.
Documentation of assessments, progress notes, and care plans will be completed accurately and timely within electronic medical record systems, aligning with payer and regulatory requirements.
As part of CarePartners’ collaborative and compassionate culture, you will actively participate in interdisciplinary case conferences, contribute to care coordination efforts, and communicate changes in patient status promptly to the broader care team.
You will support quality improvement initiatives, follow company and industry best practices, and uphold all safety, infection control, and ethical standards while working independently in the field.
