Job Description

Stellar Senior Living is seeking a compassionate and dedicated Physical Therapist (PT) to join our Healthcare & Medical Services team, supporting residents across independent living, assisted living, and memory care communities. In this role, you will play a vital part in enhancing the lives of older adults by helping them maintain or regain mobility, independence, and overall quality of life. As a Physical Therapist with Stellar Senior Living, you will conduct thorough evaluations, develop individualized treatment plans, and deliver evidence-based interventions tailored to each resident’s unique needs and goals. You will focus on improving strength, balance, gait, pain management, and functional abilities to support safe ambulation, transfers, and daily activities. Working within a collaborative, interdisciplinary environment, you will partner closely with nurses, physicians, occupational and speech therapists, caregivers, and family members to provide coordinated, resident-centered care. You will educate residents and their families on home exercise programs, the proper use of assistive devices, and fall-prevention strategies, empowering them to actively participate in their own health and wellness. Accurate, timely documentation of assessments, progress notes, and treatment outcomes is essential, as is adherence to all state and federal regulations, professional standards, and organizational policies.
