Job Description
Cardinal Hill Rehabilitation Hospital, part of Encompass Health, is seeking a dedicated Physical Therapist to join our Healthcare & Medical Services team and provide exceptional, patient-centered rehabilitative care. In this role, you will evaluate patients with a wide range of orthopedic, neurological, and medically complex conditions, and develop individualized treatment plans that promote functional recovery, maximize independence, and improve quality of life. Drawing on evidence-based practice, you will perform comprehensive physical therapy assessments, set measurable clinical goals in collaboration with the patient, family, and interdisciplinary team, and deliver hands-on interventions including therapeutic exercise, gait training, balance and coordination activities, manual therapy, and patient and caregiver education. Working in our dynamic inpatient rehabilitation environment, you will document assessments, progress notes, and care plans accurately and timely in the electronic medical record, ensuring regulatory and accreditation standards are met. You will collaborate closely with physicians, nurses, occupational and speech therapists, case managers, and other clinical staff to coordinate care, contribute to interdisciplinary team conferences, and support safe discharge planning and community reintegration. You will consistently monitor patient progress, adjust treatment plans as needed, and communicate clearly with patients and families to support understanding, motivation, and adherence to home exercise and safety guidelines.
