Job Description

Mantey Heights Rehabilitation & Care Center, part of Stellar Living in Grand Junction, CO, is seeking a compassionate and resident-focused Occupational Therapist (OT) to join our Healthcare & Medical Services team.

In this role, you will provide high-quality, evidence-based occupational therapy services to adults and seniors in a post-acute, long-term care, and assisted living environment.

You will evaluate residents’ functional abilities, develop individualized treatment plans, and deliver therapeutic interventions that promote independence, safety, and meaningful engagement in daily activities.

Working closely with an interdisciplinary care team that includes nursing, physical therapy, speech therapy, social services, and families, you will focus on enhancing each resident’s quality of life, consistent with Stellar Living’s culture of family, wellness, and engagement.

Responsibilities include completing comprehensive assessments, setting measurable goals, providing skilled treatments such as ADL retraining, adaptive equipment training, cognitive and perceptual remediation, balance and coordination training, and home safety recommendations, and documenting all services in a timely, accurate manner in accordance with state, federal, and facility standards.

You will educate residents and their families on home programs, energy conservation, fall prevention, and safe caregiving techniques, and collaborate with the care team on discharge planning and transitions of care.

The ideal candidate is professional, empathetic, and organized, with strong clinical reasoning skills and the ability to tailor interventions to the unique needs and preferences of each resident.
