About This Role
You'll work directly with senior residents at a skilled nursing facility, conducting assessments and designing personalized therapy plans to improve their mobility and daily functioning. Your days involve hands-on treatment sessions, documentation, and regular communication with nurses, doctors, and other care team members to coordinate comprehensive patient care. This is a facility-based role where you'll spend most of your time on-site working with residents in various care settings.
