Shift: 5x8 Days (08:00–16:30) Start date: 05/25/2026 Setting: Skilled Nursing Facility Responsibilities Conduct initial and ongoing PT assessments for residents in a skilled nursing setting. Develop and implement individualized treatment plans to improve mobility, strength, endurance, balance, and functional independence. Provide therapeutic exercises, gait training, balance activities, ROM work, and modality-based interventions as prescribed. Document patient progress, modify plans of care as needed, and communicate with the interdisciplinary team. Educate residents and families on home exercise programs, safety, fall prevention, and activity modification. Ensure safety, proper use of equipment, and adherence to regulatory and documentation standards. Requirements Active Physical Therapist license in California (or eligibility to obtain CA licensure). Doctor of Physical Therapy (DPT) degree or equivalent. Experience in geriatric or skilled nursing facility rehabilitation preferred; ability to work with older adult populations. Strong communication and documentation skills; proficient with standard electronic medical record systems. Ability to work a 36-hour per week schedule in a skilled nursing facility environment; flexible approach to patient needs. Willingness to travel for a travel assignment and adhere to assignment terms.
