Job Description
Life Care Centers is seeking a dedicated Physical Therapist to join our Healthcare & Medical Services team, providing compassionate, evidence-based rehabilitation services to primarily senior residents in our Life Care Centers facility.
In this role, you will evaluate patients, develop individualized treatment plans, and deliver skilled interventions designed to restore function, reduce pain, and promote maximum independence and quality of life.
You will work closely with an interdisciplinary team—including nursing, occupational therapy, speech therapy, social services, and physicians—to coordinate comprehensive patient-first care in a family-focused environment.
Responsibilities include performing detailed physical therapy evaluations, setting functional and measurable goals, implementing therapeutic exercise programs, manual therapy, gait and balance training, and the use of appropriate modalities.
You will regularly document patient progress in the electronic medical record, communicate clearly with patients and their families about treatment plans and home exercises, and participate in care plan meetings and discharge planning to support safe transitions of care.
