Stellar Senior Living is hiring a Certified Occupational Therapist Assistant to provide hands-on therapy services across independent living, assisted living, and memory care units in Fruita. You'll work under an occupational therapist's supervision, delivering treatments that help residents maintain independence and improve daily functioning. Your days will involve direct patient interaction—assisting with therapeutic activities, monitoring progress, documenting outcomes, and adapting interventions based on resident needs. In this role, you will work under the supervision of a licensed Occupational Therapist to deliver high-quality, person-centered therapeutic services that promote independence, safety, and quality of life for residents in independent living, assisted living, and memory care settings. You will assist in implementing individualized treatment plans, focusing on functional daily activities such as self-care, mobility, cognitive tasks, and home management, while carefully documenting progress and communicating updates to the interdisciplinary care team. As a COTA at Stellar Senior Living, you will conduct therapeutic activities and exercises, provide hands-on support and cueing, and adapt tasks and environments to meet residents’ physical, cognitive, and emotional needs. You will educate residents and families on safe techniques, use of adaptive equipment, and strategies to maintain or improve functional abilities. Collaboration is central to this role; you will partner closely with nurses, caregivers, therapists, and physicians to support coordinated care, participate in care conferences, and contribute to discharge and transition planning when applicable. You will also play an important role in safety and fall-prevention efforts, assisting with environmental assessments, recommending modifications, and training staff in safe resident-handling and positioning techniques. Accurate and timely clinical documentation, adherence to all regulatory and company standards, and support of quality improvement initiatives are core expectations.
