Job Description

Life Care Centers is seeking a compassionate and patient-focused Licensed Physical Therapist Assistant to join our Healthcare & Medical Services team in our senior living and rehabilitation community. In this role, you will work under the direction and supervision of a licensed Physical Therapist to deliver high-quality, evidence-based rehabilitative care to primarily geriatric patients. You will assist in implementing individualized treatment plans designed to improve mobility, strength, balance, functional independence, and overall quality of life. As a Licensed Physical Therapist Assistant at Life Care Centers, you will provide hands-on therapeutic interventions including therapeutic exercises, functional mobility training, gait and balance training, and use of assistive devices. You will observe and document patient progress, communicate changes in condition, and collaborate closely with the supervising Physical Therapist, nursing staff, and other members of the interdisciplinary care team to ensure coordinated, patient-first care. Accurate, timely documentation in electronic health records and adherence to all regulatory, safety, and infection control standards will be an essential part of your daily work. You will play a key role in patient and family education, reinforcing home exercise programs, safe transfer techniques, fall-prevention strategies, and proper use of equipment. With a strong emphasis on dignity and respect, you will support patients through their rehabilitation journey, encouraging participation, monitoring tolerance to treatment, and adapting activities under the direction of the Physical Therapist to meet each patient’s changing needs.
