Job Description

CarePartners seeks a Physical Therapist Case Manager to support members in the Franklin area through coordinated, patient-centered home-based care.

In this role, you will manage a caseload of health plan members, perform functional assessments, develop and implement care plans, and collaborate closely with physicians, nurses, and community providers.

You will monitor progress, adjust treatment as needed, and ensure services are efficient, high quality, and aligned with care goals.

Ideal candidates bring strong clinical skills, excellent communication, and a passion for improving community health and member outcomes.
