Job Description
CarePartners, a mission-driven health insurance company, seeks a part-time Physical Therapist for Home Health in Franklin.
In this Healthcare & Medical Services role, you’ll provide in-home assessments, develop and implement individualized treatment plans, and promote safe mobility and functional independence.
Collaborate with an interdisciplinary team to ensure coordinated, patient-centered care.
Ideal candidates are licensed PTs with strong clinical skills, excellent communication, and a compassionate approach aligned with our focus on community health improvement and high-quality, efficient care delivery.
