Shift: Days, 8-hour shifts (40 hours/week) Start date: 07/06/2026 Work setting: Home health care in patients’ homes Responsibilities Plans, organizes and directs home care services to meet the needs of patients in their homes. Manages a patient caseload in an effective and efficient manner. Provides patient care independently, but also collaborates with other core disciplines in accordance with the patient’s plan of care to meet care goals. Functions as a resource to patients, family members, and other care team members. Conducts evaluations, develops treatment plans, administers PT interventions, and documents progress. Maintains safety and quality standards, while communicating progress and barriers with the care team. Requirements Ability to provide PT services in a home-based setting and manage a patient caseload. Strong communication skills to educate patients and coordinate with families and other team members. Willingness to travel to patients’ homes within the service area. Ability to document care, progress notes, and outcomes accurately. Demonstrated ability to work independently and as part of a multidisciplinary team. Minimum 2 years of recent acute-care experience in the specialty.
BLS required. ACLS, PALS, or specialty certs depending on unit.
Ability to stand for long shifts, lift up to 50 lbs, and assist with patient transfers.
