CarePartners is seeking a compassionate and skilled Physical Therapist for our Home Health CarePartners team, working within the Healthcare & Medical Services department. In this role, you will provide one-on-one, evidence-based physical therapy services to adult and geriatric patients in their homes across local Connecticut communities. You will evaluate patients’ functional status, develop and implement individualized plans of care, and deliver therapeutic interventions that promote safety, mobility, independence, and quality of life. You will collaborate closely with physicians, nurses, care managers, and other allied health professionals within CarePartners’ coordinated provider network to ensure seamless, patient-centered care. As a Physical Therapist in home health, you will perform comprehensive initial and ongoing assessments, establish measurable functional goals, and adjust treatment plans as patients progress. You will educate patients and caregivers on home exercise programs, safe mobility and transfer techniques, fall prevention, and use of assistive devices, helping patients remain safely at home whenever possible. Accurate, timely documentation in accordance with home health and regulatory standards will be an integral part of your daily work. You will embody CarePartners’ culture of collaboration, compassion, and community health improvement, serving as both a clinical expert and a trusted resource in the home setting. You will coordinate closely with the broader CarePartners network to support efficient healthcare service delivery, proactively communicate changes in patient status, and contribute to care planning that reduces avoidable hospitalizations and supports successful transitions of care.
