CarePartners of Connecticut is seeking a compassionate, self-directed Physical Therapist for our Home Health CarePartners team within our Healthcare & Medical Services department.

In this role, you will deliver high-quality, evidence-based physical therapy services to patients in their homes across the local Connecticut community, helping them regain mobility, function, and independence while supporting CarePartners’ mission of improving community health through coordinated, patient-centered care.

Working closely with physicians, nurses, case managers, and other allied health professionals, you will evaluate patients’ physical function and home environment, develop individualized treatment plans, and provide hands-on therapeutic interventions including therapeutic exercise, gait and balance training, functional mobility training, and patient and caregiver education.

You will regularly reassess progress, adjust care plans, and document all visits and outcomes in accordance with regulatory, payer, and CarePartners standards, ensuring clear, timely, and accurate clinical records in our electronic health record systems.

The position requires strong clinical judgment, excellent communication skills, and the ability to work both independently in the field and collaboratively as part of an interdisciplinary care team.

As a representative of CarePartners in the home, you will build trusting relationships with patients, families, and caregivers, respect cultural and individual differences, and advocate for resources and services that support optimal health outcomes.

You will coordinate closely with our provider network to align home health therapy with overall care plans, contributing to reduced hospitalizations, improved recovery, and greater patient satisfaction.
