About This Role
You'll travel to patients' homes throughout the Skyland area to deliver one-on-one physical therapy sessions focused on restoring mobility and independence for adults and older adults. Your days involve assessing patients, designing personalized treatment plans, and conducting hands-on therapy while managing pain and functional goals. You'll coordinate regularly with nurses, doctors, and family caregivers to adjust treatment as patients progress, educate patients on exercises they can do independently, and maintain detailed clinical documentation. This role suits therapists who prefer direct patient relationships over clinic settings and work well with older populations. You'll need a valid PT license, strong communication skills, and the ability to adapt treatment plans based on home environments. Travel between appointments is part of the job.
