Job Description
St.
David's Medical Center is seeking a Home Health Physical Therapist to provide one-on-one rehabilitative care to patients in their homes across the Austin area.
In this role, you will evaluate functional status, develop and implement evidence-based treatment plans, educate patients and families, and collaborate closely with physicians, nurses, and interdisciplinary team members.
You will document care in the EMR, ensure regulatory compliance, and support safe transitions from hospital to home, reflecting our culture of compassionate, patient-centered excellence and teamwork.
