Job Description
Methodist Hill Cntry Home Care, part of Hill Country Supportive Care and Methodist Physician Practices, is seeking a PRN Home Health Physical Therapist to provide high-quality, compassionate rehabilitation services to patients in their homes. In this role, you will partner closely with an interdisciplinary team to support patients living with serious or chronic illness, focusing not only on physical function and safety, but also on enhancing overall quality of life. Working within a holistic, patient-centered model, you will complete comprehensive physical therapy evaluations in the home setting, develop individualized plans of care, and deliver evidence‑based interventions to improve mobility, strength, balance, pain, and endurance. You will educate patients and families on home exercise programs, safe transfer techniques, use of assistive devices, fall prevention strategies, and environmental modifications that support independence. You will coordinate care and communicate regularly with nurses, physicians, and other team members to ensure therapy goals are aligned with the patient’s overall palliative or supportive care plan. As a PRN clinician, you will have flexible, as‑needed scheduling while still being an integral part of a supportive, mission‑driven team that values empathy, clinical excellence, and collaboration. You will be expected to manage a home‑based caseload efficiently, travel between patient homes, complete timely and accurate documentation in the electronic medical record, and adhere to all regulatory, safety, and infection‑control standards. The ideal candidate is a licensed Physical Therapist with strong clinical skills in adult and geriatric rehabilitation, experience or interest in home health or serious‑illness care, and a commitment to treating patients and families with dignity, respect, and compassion. You should be comfortable working independently in the field, making sound clinical judgments, and advocating for the functional needs and goals of each patient. Requirements
Active PT license in good standing in the state of employment
1+ year of relevant experience; ability to onboard quickly.
BLS required. Specialty certs based on unit.
Ability to stand for long shifts, lift up to 50 lbs, and assist with patient transfers.
