Job Description
CarePartners of Connecticut, a health insurance company dedicated to improving community health through coordinated, patient-centered care, is seeking a Part-Time Physical Therapist for Home Health services in Franklin. In this role, you will provide evidence-based physical therapy to adult and geriatric members in their homes, helping them improve mobility, function, and quality of life while safely remaining in the community. Working within the Healthcare & Medical Services department, you will complete comprehensive in-home evaluations, develop individualized treatment plans, and deliver hands-on therapy tailored to each patient’s goals, functional level, and home environment. You will collaborate closely with CarePartners’ network of providers, including physicians, nurses, case managers, and other allied health professionals, to coordinate care, support safe transitions from hospital or rehab to home, and ensure that treatment plans are integrated with each member’s overall care strategy. You will document assessments, goals, interventions, and outcomes accurately and promptly in the electronic health record, adhering to regulatory, payer, and organizational standards. You will educate patients and caregivers on home exercise programs, fall prevention, safe mobility techniques, use of assistive devices, and energy-conservation strategies, promoting independence and long-term wellness. This position is ideal for a clinically strong, compassionate Physical Therapist who values autonomy in the field combined with the support of a coordinated health system. You will manage a part-time home health caseload in the Franklin area, organizing your schedule to complete timely visits, reassessments, and discharges while meeting quality, compliance, and productivity expectations. You will participate in interdisciplinary case conferences, contribute to care planning, and communicate changes in patient status to the broader care team. You will uphold CarePartners’ culture of collaboration, compassion, and patient-centered care by building trusting relationships with patients, families, and providers, and by advocating for services and equipment that support safe, independent living at home. The Physical Therapist Home Health Franklin Part Time role offers the opportunity to directly impact community health outcomes while enjoying flexible, field-based work. You will stay current with clinical best practices, integrate outcome measures into your evaluations and progress notes, and support continuous improvement initiatives across the home health network.
