Job Description
CarePartners is seeking a compassionate and self-directed Physical Therapist for our Home Health team. In this role, you will provide one-on-one, skilled physical therapy services to patients in their homes, helping them regain mobility, prevent falls, and optimize independence following illness, injury, or surgery. Working within the Healthcare & Medical Services department, you will collaborate closely with physicians, nurses, case managers, and other members of our partnered provider network to develop and implement individualized plans of care. As a Physical Therapist Home Health CarePartners clinician, you will perform comprehensive patient assessments, establish functional and measurable treatment goals, and deliver evidence-based therapeutic interventions, including therapeutic exercise, gait and balance training, neuromuscular re-education, and patient and caregiver education. You will monitor clinical progress, adjust treatment plans as needed, and document all care accurately and timely in the electronic health record to support coordinated, high-quality care. Aligned with CarePartners of Connecticut’s mission to improve community health and provide coordinated care, you will play a key role in ensuring safe transitions from hospital or facility to home, reducing avoidable readmissions, and enhancing the overall patient experience. You will communicate regularly with referring providers and the broader care team to report patient status, address barriers to care, and advocate for necessary services or equipment, such as durable medical equipment or assistive devices. This position is ideal for a Physical Therapist who thrives in an autonomous, field-based environment and values meaningful, long-term relationships with patients and families. You will manage a home health caseload within an assigned territory, plan efficient daily routes, and prioritize visits based on clinical need and physician orders. You will also educate patients and caregivers on home exercise programs, safe mobility techniques, fall prevention strategies, and energy conservation, empowering them to participate actively in their recovery. As part of our home health team, you will contribute to quality improvement initiatives, follow evidence-based clinical pathways, and uphold all regulatory and payer requirements relevant to home health services. You will participate in case conferences, interdisciplinary team meetings, and ongoing professional development activities to maintain and advance your clinical skills. The ideal candidate is clinically strong in orthopedic, neurologic, and geriatric rehabilitation; comfortable working independently in the community; and motivated to support CarePartners’ commitment to providing coordinated, high-value healthcare throughout Connecticut. This is a full-time role with a primarily field/on-site work environment, requiring travel to patient homes across the designated service area.
